Your Treatment Team

Family Home Health Services is devoted to providing exceptional homecare and hospice services. Making a difference in the lives of our patients is our highest calling. We are rewarded for our services by seeing the enhanced quality of life and the physical and emotional recovery in our patients.
At Family, we do not simply assign people to your treatment team based on our judgment alone. We give you control of your recovery and the team chosen will reflect your own personal preferences, needs and treatment goals. Your caregivers specialize in four areas:
Care and Assistance
Your Treatment Team consists of experienced caregivers who will assist you with your basic needs. Should you need more support during your recovery, we will help you with personal care needs such as bathing, dressing and toileting. By giving you relief from these activities of daily living, you will be able to concentrate on what is most important – your recovery and rehabilitation.
Recovery
In a coordinated plan, your team will help guide you to reach your goals of independence. This plan is based on your own personal preferences and desires so it adheres to a pace you are comfortable and satisfied with.
Notification
Your educated caregivers will be able to notice significant changes in your condition that need to be reported to your physician. We want our patients to feel safe and secure in knowing that they are being looked after by a knowledgeable staff of professionals.
Education
We will teach you and those who care for you, how to manage your medical condition, be alert for signs and symptoms of exacerbation, assist in the administration of your medication regime and help maintain your safe independence at home.
You may receive treatment from any or all of the following experienced Family staff:
- Registered Nurses - RNs
- Licensed Practical Nurses - LPNs
- Physical, Occupational, or Speech Therapists
- Medical Social Workers
- Certified Home Health Aides.
