Patient Centered Approach to Care

Family Health Services realizes that every patient we care for is unique in several ways. No two medical conditions are exactly the same and every patient experiences their condition differently. All individual patients have personal preferences and desires about how they should be cared for. Finally, patients have different goals for their treatment and different ideas on how those goals should be achieved. Shouldn’t you expect your home health care to address these unique needs?

A Plan of Care Developed by the Patient

That is why we work with our patients in creating a unique Plan of Care that is tailored to meet and support their treatment goals. Rather than making decisions for our patients, we ask each individual what he or she would like to accomplish during the course of the care and how we can help them achieve those goals. Then, we develop a Plan of Care which best utilizes the capabilities of our interdisciplinary treatment team. It is this "patient centered" approach which distinguishes Family from other homecare providers.

Take Charge of Your Recovery

By shifting the focus from a clinician driven treatment plan to a patient driven plan, we empower the patient to have:

• A stake in rehabilitation and an ownership in recovery
• Control over the way in which they are cared for based on their personal preferences
• A responsibility to become an active, rather than passive participant in the road to independence
