Referral Information



How to Make A Referral

We value our referral services and thank them for their support by striving to make their lives easier. We understand that completing a referral often involves multiple phone calls, tedious paperwork and a considerable amount of time. At Family Home Health Services, it is our goal to make the referral process as smooth and straightforward as possible.

Once a patient has been referred, our skilled nurses can conduct an onsite visit and perform a Transitional Assessment which evaluates the patient’s condition.

The Transitional Assessment consists of three simple steps:

  • Interview the patient, family, treating therapists and staff
  • Review the relevant medical records
  • Obtain the medical, diagnostic, historical and social information necessary to prepare the patient for admission to our home health, hospice or private duty programs.

We believe that the referral process does not end after a name, address and diagnosis is exchanged. It is our pleasure to regularly contact each referral source with an update on the patient’s status after our services have begun.

Our Referral Source Satisfaction Survey Results

In order to maintain a consistent quality of excellence, we send satisfaction surveys to our referral sources twice a year. The results of these surveys allow us to improve our intake process to make it increasingly user-friendly for those who entrust us with the care of their patients.

We take pride in the customer satisfaction we give our referral sources and that is why our surveys consistently show a 95 to 100% satisfaction rate for our services.
