What are the qualifications for Home Health Services?
Do You Qualify for Home Health Care?

We accept payment for services from Medicare, Medicaid, Worker’s Compensation, private insurance or private pay.


The following items are required for Medicare to pay for home health care services:

  • You are homebound. This means that due to your illness or injury, you are unable to leave your home without a considerable, taxing effort and absences are short and infrequent.
  • You have had a recent illness or injury (or worsening of a condition) which requires the skilled care of a Registered Nurse, Physical Therapist, or Speech Language Pathologist.
  • You are under the care of a doctor who has ordered the treatment or services we are providing.



What are the qualifications for Hospice Services?
Who is eligible for Hospice under the Medicare benefit?

The following items are necessary in order for Medicare to cover your Hospice Services:

  • You must be eligible for Medicare Part A.
  • A Physician must sign a certification stating that you have an illness with a life expectancy of 6 months or less.
  • Upon review of your case, a Hospice Medical Director must agree with the prognosis of the certifying Physician.
  • You must sign a consent agreeing to Hospice care rather than aggressive treatment for the terminal diagnosis.
  • The Hospice Provider you choose must be Medicare Certified (as Family Centered Hospice is).
