The focus of hospice care is to provide pain and symptom management, and spiritual and emotional support, so that patients can maintain their dignity and live as fully as possible by spending their remaining days in comfort. The Family Centered Hospice Inpatient Unit at Alden Town Manor is dedicated to providing expert clinical management to individuals who are determined to be appropriate for this level of hospice care.

Times When Inpatient Care is Appropriate

Immediately following discharge from the hospital when an individual has complex discharge needs that include adjustments in symptom management procedures and would benefit from the assistance of an interdisciplinary hospice team to make the transition safe and effective
- For individuals who need short-term intensive hospice care (generally three to four days) to manage acute medical, spiritual or psychosocial symptoms that cannot be easily managed in another setting.
- When care can no longer be provided in the home or nursing home setting.
The Care Provided

Individuals and their families are offered a full range of services to meet the physical, psychosocial, emotional and spiritual needs that arise during a terminal illness.

Care focuses on pain control, symptom management and emotional support.
- The Hospice Inpatient Unit is staffed by highly trained Family Centered Hospice professionals, including physicians, nurses, certified nurses’ aides, social workers, chaplains, volunteers and bereavement counselors who speak a variety of languages and have sensitivity to a variety of cultures. The individual’s personal physician is encouraged to visit and follow the hospice patient.

Spacious rooms featuring accommodations and furnishings that offer a calming, home-like environment, including recliners for overnight stays.

A setting conducive to educating and training family members and caregivers on how to care for the patient if there is a desire to transition to another setting.

A private wing housed on the main floor of a comprehensive rehabilitation and health care center, providing additional medical assistance if needed.

Family kitchen and lounge areas to provide quiet, comfortable spaces to gather.

Free wireless Internet access (WiFi).

Unrestricted visiting hours for family and loved ones.
Financial Considerations

The Medicare Hospice Benefit covers the cost of the inpatient level of hospice care. This same coverage is available through Medicaid.

Many private insurance companies have coverage for inpatient hospice care.

Hospice services are provided regardless of age, gender, race or religion.
